Pilocarpine HCl
Indications/Uses
Control of IOP in chronic simple glaucoma. May be used alone in acute glaucoma prior to emergency surgery, or in combination w/ other miotics or carbonic anhydrase inhibitors.
Dosage/Direction for Use
1 or 2 drops bd-qds. May be necessary to repeat instillations up to 6 times daily in acute glaucoma.
Contraindications
Hypersensitivity. Conditions where pupillary constriction is undesirable eg, acute iritis or anterior uveitis. In case of glaucoma due to pupillary block, as long as no accomplished thorough exam of retina & its periphery.
Special Precautions
Not for inj or ingestion. Only administer to myopic persons after exam of retina & its periphery. Reports of retinal detachment when used in susceptible individuals eg, young patients w/ myopia or patients w/ history of retinal detachment. Be attentive to possible systemic effects caused by resorption through general circulation. Caution in patients w/ acute cardiac failure, bronchial asthma, peptic ulcer, Parkinson's disease, urinary tract obstruction & HTN. Do not abruptly interrupt treatment. Risk of paradoxical rise in IOP in patients w/ severely compromised trabecular outflow. May cause blurred vision & difficulty w/ dark adaptation. Darkly pigmented iris may require higher conc or more frequent administration. Contains benzalkonium Cl. Remove contact lenses before instillation & reinsert 15 min afterwards. Keep eyelid closed for 2 min & close lachrymal canal w/ finger for 2 min after instillation to reduce systemic resorption. Major influence on the ability to drive & use machines. Avoid use during pregnancy. Lactation.
Adverse Reactions
Headache; blurred vision. Dizziness; visual impairment, reduced visual acuity, eye pain, photopsia, myodesopsia, eye irritation, ocular hyperaemia; nausea.
Drug Interactions
Decreased effect w/ topical NSAIDs. Increased risk of conduction disturbances w/ co-administration of oral pilocarpine & β-adrenergic antagonists. Can negatively affect the efficacy of ophth adrenergics (eg, epinephrine) resulting in increased myopia. Diminished efficacy of ophth latanoprost. Concurrent use w/ ophth sulfacetamide may result in precipitation of pilocarpine on eye surface. Risk for potentiation of curarising agents. Administer at least 5 min apart if >1 eye prep is being used.
